Practice Fireworks Safety and Prevent Cataracts

July 1, 2024 by Emily Grant

Eye trauma and injuries can cause vision loss or blindness, so protect your eyes by watching a professional fireworks display.

Fireworks Are a Common Cause of Summer Injuries

It is exciting to see explosions of bright colors in the night sky! Americans have been using fireworks to celebrate freedom since the first organized Independence Day celebration on July 4, 1777, in Philadelphia. Although firecrackers and sparklers play a major role in summer celebrations, they pose a critical risk to eye safety.

Fireworks cause thousands of injuries each year, and the majority of accidents occur from May to July. According to the American Academy of Ophthalmology, almost 300 people go to the emergency room each year due to fireworks-related eye injuries during the weeks surrounding July 4.

Hospital emergency rooms treated 10,200 fireworks injuries in 2022, and 73 percent happened around the Fourth of July. Burns were the most common injury, and 28 percent of all injuries were to children younger than 15.

Body parts most often injured:

  • Fingers and hands (29 percent)
  • Face, head and ears (19 percent)
  • Legs (19 percent)
  • Eyes (16 percent)
  • Torso (12 percent)
  • Arms (5 percent)

How Can Fireworks Damage the Eyes?

Fireworks can damage any part of the body, but the eye is susceptible to chemical, thermal or blast injuries. Injuries to the eye can range from minor scratches on the cornea and objects stuck in the conjunctiva to cuts on the eyelid, retinal detachment, broken eye socket and penetrating eye wounds. Explosives can also cause burns around the eyes, facial lacerations, fractures and scarring.

“Chemicals found within fireworks cause the amazing display of colors and explosions that we see during celebrations, but these can be harmful to the eye,” said Grayson W. Armstrong, MD, MPH, director of ophthalmology emergency services at Mass Eye and Ear and board member of the American Society of Ophthalmic Trauma. “The eye needs to remain at a normal pH, and if there is acidic or basic material exposed to the eye, it can lead to chemical burns and sometimes long-term problems such as limbal stem cell deficiency, scarring, cataract, glaucoma and even retinal toxicity.”

Protect Your Vision and Prevent Ocular Damage

Fireworks may be legal, but it does not mean they are safe. Legally purchased fireworks can contain modified fuses, dangerous chemicals and unsafe levels of explosive materials. Explosives can misfire, ignite all at once or discharge in the opposite direction.

Watching a show in the crowd also poses some risk because 65 percent of fireworks injuries are to bystanders who were too close to the action. Many believe sparklers are safe for kids, but they can reach temperatures up to 2,000 degrees and result in over 600 injuries annually.

You can protect your eyes by following these safety tips:

  • Attend a professional fireworks show.
  • Purchase non-toxic glow sticks for children and grandchildren to wear during the show.
  • View fireworks from at least 500 feet away.
  • Do not touch unexploded fireworks.

Cataracts May Impede Your View of Colorful Fireworks

Do you miss the vivid pop of brilliant color you used to enjoy during fireworks displays? If you’re noticing colors don’t appear as bright as they used to, you may be developing cataracts, or a clouding of the eye lens. Having cataracts can affect your vision by making vivid hues look yellow or faded.

Cataracts are a normal part of the aging process. However, eye trauma or eye injury can also cause cataracts. Up to 65 percent of eye injuries lead to cataract formation and can cause short and long-term vision loss.

Traumatic cataracts can form quickly or develop slowly over time. Acute injuries like blunt or penetrating trauma, electric shock or chemical injuries can damage or rupture the lens, leading to partial or complete cataracts and blindness.

You should call your ophthalmologist right away if you experience symptoms of cataracts:

  • Blurred vision
  • Double vision
  • Seeing halos around lights
  • Glare at night
  • Fading or yellowing of colors

Cataract Surgery Can Restore Clear Vision and Your Ability to See Brilliant Color

Cataract surgery can restore most cataract-related vision problems. It is one of the safest and most common procedures, and has a success rate of 97 percent or higher when performed in appropriate settings.

Cataract surgery is usually an outpatient procedure and requires a short recovery period. It usually takes less than 30 minutes, and patients can return home after a few hours.

The surgeon will remove the cloudy lens and replace it with a customized artificial lens called an intraocular lens (IOL). The incision is so small that stitches are rarely necessary. Most patients find they can resume their normal activities the following day.

Schedule a Comprehensive Eye Exam Before the End of the Year

All adults over 40 should have a comprehensive eye exam every year. During a vision examination, the optometrist will assess your visual acuity, check for any refractive errors and examine for all types of cataracts, glaucoma, diabetic retinopathy and macular degeneration.

Have you been putting off vision testing or an eye procedure? The second half of the year is a good time to schedule procedures because you may have already reached your deductible. Call your insurance company to find out how close you are to meeting your deductible and then call your eye doctor. Your eye procedure may cost less than you thought.

Warm-ups Help Prevent Joint Injury When Playing Tennis and Golf

July 1, 2024 by Emily Grant

Tennis and golf can improve your fitness, but it is essential to warm up, stretch and wear proper footwear to protect your joints and prevent overuse injury.

Summer Sports Enhance Physical and Mental Wellness

Summer is an ideal time to try outdoor sports like tennis and golf. Engaging in these activities can provide a range of advantages, such as reducing heart rate and blood pressure, enhancing cognitive function and boosting overall mood and positivity. Even though sports like golf are low impact, you still can sustain injuries like sprains, fractures and strains.

Arthritis Can Increase the Risk of Overuse Injury

You are even more prone to injury if you suffer from any form of arthritis. Repetitive motions and stress can cause joint damage to the shoulders, hips, knees and elbows.

Repetitive stress injuries (RSI) can occur from repeatedly swinging a golf club or hitting a tennis ball, causing pain and affecting the joints in the shoulder, elbow and knee. Cumulative trauma disorder (CTD) is a small injury to a ligament, tendon, muscle or bone and occurs when exerting too hard or not allowing your muscles to recover.

Common Overuse Injuries

Tennis and golf can make you more susceptible to overuse injuries, such as the following:

  • Shin splints — Shin splints are overuse injuries that cause inflammation and tenderness in the inner shin. The tissue around the shin can throb or feel like dull pain.
  • Runner’s knee — This condition does not just affect runners. Playing a sport on uneven or hard surfaces can cause swelling and pain around the patella (or kneecap).
  • Shoulder impingement — When a tendon in the shoulder rubs against surrounding tissue or bone, it can cause persistent pain. Shoulder impingement can affect the rotator cuff and lead to weakness and decreased mobility.
  • Tennis/golfer’s elbow — Gripping a tennis racket or golf club during a swing can increase muscle strain and cause tendons to get microscopic tears. Inflammation builds over time and causes the tendon to break down.

Proper Equipment Protects Your Joints during Golf and Tennis

Quality footwear can help reduce your risk of joint injury during sports. Golf shoes are typically more rigid than athletic shoes. The grip — provided by a unique sole design or detachable studs crafted from durable materials like hard rubber, plastic or alternative non-metal options — ensures the foot stays secure for an extended period.

Tennis requires different footwear. Because of all the lateral movement of tennis, you need shoes with a high level of support and cushioning. The soles of the shoes are flat to keep the foot stable and prevent ankle rolling and knee injury.

Choosing lighter tennis rackets and golf clubs can help reduce the risk of tennis elbow, wrist tendonitis and rotator cuff tears. Using a bigger racket head can also help by reducing strain on the arm when hitting the ball in the middle of the racket, rather than the edges of the frame.

Gripping too tightly to clubs and rackets can increase vibration load on your arm, so remind yourself to loosen your grip. Reducing string tension can also make a significant difference.

Warm Up and Stretch Before You Begin Playing Sports

Doing simple stretches before exercise will help protect your joints, reduce pain and prevent injury.

Try these easy warm-ups before stepping out on the golf course or tennis court:

  1. Shoulder rolls
  2. Leg swings
  3. Torso twists
  4. Toe touches
  5. Body weight squats
  6. Split stance rotations
  7. Speed swings
  8. Standing calf stretch
  9. Hamstring stretch
  10. Hip flexor stretch
  11. Arm circles
  12. Wrist extensor stretch

It is also essential to take regular breaks during exercise to allow your knee, hip and shoulder joints to rest. You should also be intentional about hydrating your body and wearing SPF protection for your skin.

How to Recover from Overuse Injuries

Rest is the best prescription for recovering from an overuse injury. Avoid activities that cause discomfort and may injure you further. You may need physical therapy to strengthen the affected muscles and regain range of motion.

How Soon Can You Resume Physical Activity after Joint Surgery?

Many people choose joint surgery to help them regain mobility they have lost due to joint deterioration. It is imperative to allow the affected joint adequate time to heal. Jonathan Levy, MD, director of the Levy Shoulder Center at the Paley Orthopedic and Spine Institute in Boca Raton, Florida, led two studies about returning to golf, pickleball and tennis after shoulder surgery. Dr. Levy found it took most patients about six months to play at their former level.

“It’s not an extraordinarily painful recovery,” Dr. Levy said. “If people are taking pain medication, it’s for a very short time. It’s just going through the stages of recovery where you go through healing, stretching and then strength recovery and return to activity.”

Returning to play sports often depends on how much stress you put on the new joint and the type of sport you play. Golf usually causes less strain than activities like pickleball and tennis or biking and skiing.

Make an Appointment with Your Orthopedist Before the End of the Year

If you are suffering from persistent pain caused by overuse, it is advisable to contact either your orthopedist or orthopedic surgeon. You may need physical therapy, medication or a small procedure to alleviate discomfort. You may also be a candidate for total joint replacement.

Procedures like rotator cuff surgery or total knee or total hip replacement surgery may help you regain or improve mobility and allow you to resume the daily activities you previously enjoyed. Following joint replacement surgery, numerous patients report experiencing a notable improvement in their quality of life, as they regain a sense of independence.

Call today to make an appointment before the end of the year. You may want to call your insurance company to ask if you have already met your deductible. Your out-of-pocket costs may be less than you anticipated, so do not delay in scheduling a consultation.

Stool Tests May Require Colonoscopy Follow-Up

July 1, 2024 by Emily Grant

A recent study found that fewer than half of patients who had an abnormal stool-based colon cancer screening test completed a follow-up colonoscopy within six months.

Colon Cancer Screening Tests Vary in Effectiveness and Accuracy

It’s always good to have options when making important decisions. When it comes to testing for colorectal cancer (CRC), you have many choices. There are several types of screenings, from stool tests to colonoscopy. However, some screenings are more thorough and comprehensive than others.

Stool-based screening tests (SBTs) are non-invasive and efficient, but a positive stool test requires a follow-up colonoscopy. A new study published in JAMA Network Open found that 52 percent of patients did not follow through with colon cancer screening recommendations when blood was found in their stool samples.

Study Finds Poor Follow-Through with Positive Stool Test Results

Elizabeth L. Ciemins, PhD, MPH, from the American Medical Group Association in Alexandria, Virginia, led a quality improvement study to analyze whether screening-eligible adults chose to get a follow-up colonoscopy after an abnormal stool test.

Dr. Ceimens and her research team looked at data from 38 healthcare organizations for 20,581 patients aged 50 to 75 years who received abnormal results from a colorectal cancer SBT. They looked at metrics including race, ethnicity, sex and insurance. The team found 47.9 percent had a follow-up colonoscopy within a median follow-up time of 53 days.

The study found follow-up colonoscopy screening rates were highest among White patients, at 49 percent. Rates were lower among Black patients (37.1 percent), Hispanic patients (38.4 percent) and patients with Medicare and Medicaid (49.2 and 39.2 percent, respectively).

“Use of SBTs may increase overall screening rates, but abnormal results must be followed up with a colonoscopy to diagnose CRC — ideally as soon as possible, but definitely within the six months after an abnormal test result,” the research authors wrote.

Advantages of Colonoscopy Over a Stool-Based Test

Stool-based tests offer many advantages in colon cancer screening because patients can perform them at home. SBTs do not require fasting, bowel prep, transportation or sedation, and patients do not need to take time off from work.

However, SBTs, like fecal immunochemical testing (FIT), guaiac-based fecal occult blood test (gFOBT) and multi-target stool DNA (mt-sDNA) tests, detect blood or abnormal chemicals in the stool. They do not identify the source. Patients should follow up with a colonoscopy within six months of a positive SBT. If the stool test is negative, it needs to be repeated in one to three years, depending on the test performed.

Another limitation of SBTs is that they can be inaccurate. One study found that 10 percent of fecal immunochemical tests could not be processed due to patient errors in collecting the specimen.

“We hope to learn more about where the blame lies,” Rasmi Nair, assistant professor in the Peter O’Donnell Jr. School of Public Health at UT Southwestern Medical Center in Dallas, told Healio. “Is it more patient-related? Did the patient not understand the instructions with regards to FIT or was it more system- or provider-related? Although our research is great at identifying the issue, we still are not able to pinpoint the exact problem.”

A visual exam like colonoscopy can detect and even prevent colon cancer because a doctor visually inspects the lining of the colon with a scope and can remove abnormal tissue called precancerous polyps during the exam. A negative colonoscopy often means the patient does not have to repeat the test for another 10 years. Colonoscopy is a smart option for many patients due to its accuracy and less frequent need for repetition compared to other methods.

Tips to Improve Your Colonoscopy Prep and Exam

Are you concerned about getting a colonoscopy? Or perhaps you have recently received an abnormal SBT and you’re wondering if you should schedule a colonoscopy or skip it.

A colonoscopy has never been easier. The new prep solutions are low volume, more palatable and easier to consume. You also only need to take one day off from work. Below are some tips to prepare for a successful colonoscopy.

The day before prep day:

  1. Eat a big breakfast, a light lunch and a snack dinner.
  2. Start a low-fiber diet three days before your colonoscopy.
  3. Start hydrating several days before prep day.

On prep day:

  1. Chill the prep beverage and drink it through a straw.
  2. Chase the prep solution with cold water.
  3. Enjoy all the clear liquids you like, including black coffee, tea, sodas, juices, popsicles, gelatin, hard candy, Italian ice and gummy bears. (Avoid anything with red or blue dye because it may make it more difficult to examine the colon).
  4. Use baby wipes, Vaseline or diaper rash cream to give you relief.
  5. Save your favorite TV show to binge-watch on prep day.

Begin Colon Cancer Screening at Age 45 If You Are at Average Risk

Colon cancer is a leading cause of cancer death in the United States, but one-third of Americans are overdue for a colon cancer screening.

The U.S. Preventive Services Task Force recommends that all adults who are at average risk for colon cancer begin screening at age 45. Individuals with a family history of colorectal cancer, colon polyps, inflammatory bowel disease, Lynch syndrome or Familial Adenomatous Polyposis (FAP) should consider starting screening at a younger age due to their increased risk of developing colon cancer.

Talk to your doctor about when you should begin colon cancer screening. If you experience symptoms like blood in the stool, abdominal pain or cramping, nausea or unexplained weight loss, don’t delay in calling your physician. Most colon cancer is treatable when diagnosed at an early stage, so call today to make an appointment if you are due for a screening.
